Cardiac computerized tomography CT has evolved from a research tool to an important diagnostic investigation in cardiology, and is now recommended in European, US, and UK guidelines.

Technical developments have overcome the limitation of cardiac motion artifacts and made cardiac CT widely available. Coronary CT angiography CTA offers the tremendous benefit of obviating an invasive coronary angiography exam, specifically in patients with normal CTA results. With the additional features of functional cardiac and valvular assessment, and myocardial perfusion, CT has also become an alternative and complementary modality for specific questions traditionally reserved for cardiac ultrasound echocardiography or magnetic resonance imaging MRI.

Clinical Cardiac CT

This book is a comprehensive and richly-illustrated guide to cardiac CT, its current state, applications, and future directions. While the first edition of this text focused on what was then a novel instrument looking for application, this edition comes at a time where a wealth of guideline-driven, robust, and beneficial clinical applications have evolved that are enabled by an enormous and ever growing field of technology. Accordingly, the focus of the text has shifted from a technology-centric to a more patient-centric appraisal. While the specifications and capabilities of the CT system itself remain front and center as the basis for diagnostic success, much of the benefit derived from cardiac CT today comes from avant-garde technologies enabling enhanced visualization, quantitative imaging, and functional assessment, along with exciting deep learning, and artificial intelligence applications. Cardiac CT is no longer a mere tool for non-invasive coronary artery stenosis detection in the chest pain diagnostic algorithms; cardiac CT has proven its value for uses as diverse as personalized cardiovascular risk stratification, prediction, and management, diagnosing lesion-specific ischemia, guiding minimally invasive structural heart disease therapy, and planning cardiovascular surgery, among many others.

Since the advent of multislice computed tomography MSCT , remarkable technical advances have occurred. Arguably, 4-slice scanners have become obsolete, and slice systems may soon be considered antiquated with the arrival of faster slice technology. Improvements in spatial and temporal resolution have accompanied this rapid progress in technology, thus allowing for better visualization of coronary anatomy. This systematic review evaluates state-of-the-art cardiac MSCT in the diagnosis of coronary artery disease and discusses its performance relative to competing modalities.

Clinical Applications of Cardiac CT

During the past few years, cardiac CT CCT has acquired an increasingly important role as a noninvasive imaging method that allows assessment of coronary heart disease from both the morphological and the functional standpoint. It is quickly becoming a primary clinical tool for the evaluation and follow-up of various conditions related to the heart and great vessels and is providing valuable insights into the natural history of atherosclerosis. The rapid advances in CCT technology, the advent of new clinical applications, and the acquisition of data on prognostic value are just some of the reasons for the publication of this new edition of Clinical Applications of Cardiac CT , little more than 3 years after the first edition appeared. The text has been extensively revised and updated to reflect current knowledge and practice, and the structure and layout of the educational content have also been improved. The imaging targets, semeiology, technique, and clinical applications of CCT are all covered in detail, and in addition relevant information is provided on epidemiology, clinical assessment, and the role of other diagnostic modalities. This book will prove an invaluable tool for radiologists and cardiologists alike.

ECG-gated multislice CT provides a cost-effective, non-invasive technology for evaluation of the coronary arteries, as well as for additional clinical applications, which require morphological assessment of the heart and adjacent structures with simultaneous evaluation of the coronary circulation. The excellent negative predictive value of a normal coronary CTA cCTA examination excludes the presence of significant coronary disease in the symptomatic patient. Triple rule-out studies provide evaluation of the aorta and pulmonary arteries without loss of image quality in the coronary circulation. The ability to visualize surrounding vascular structures along with the coronary arteries is essential in the evaluation of coronary anomalies. Cardiac CTA is useful in non-coronary applications, including evaluation of the thoracic aorta, cardiac valves and other aspects of cardiac morphology that may require surgical or percutaneous repair. Recent advances in evaluation of coronary plaque morphology as well as myocardial perfusion will allow a more complete noninvasive cardiac assessment in the future and may provide a highly effective method of cardiac risk stratification to facilitate preventive cardiac care. The advent of multislice CT technology with ECG-gating along with innovations in x-ray tube and CT detector technology have resulted in revolutionary progress in cardiac imaging.
